You will need to make an appointment to see a GP to receive an illness benefit certificate or medical certificate for work.

If you need to extend either cert, you can submit the below form and you will contacted should a GP review be required.

A ‘Certificate of Incapacity for Work’ is needed when applying for illness benefit. Your GP submits this electronically through a secure online system. You will need to apply for Illness Benefit from the Department of Employment Affairs and Social Protection (DEASP) if you cannot work because you are sick or ill.
